Hospice UK are developing a National Transitions Community of Practice (CoP) to transform care for young people with complex and life-limiting conditions.

We recognise that the current guidance to improve services for Young People has led to many good practice examples. Integration and collaboration has enabled positive change in service delivery and improved experiences for Young People and their families. Hospice UK is pleased to be offer support to organisations to share this good work and their learning with others using ECHO methodology.This will extend reach and further influence partnerships across all care providers and key stakeholders.  

Hospice UK are seeking three organisations to participate in a three year project and become Transition ECHO hubs to transform care for young people (aged 16-25) with complex and life-limiting conditions. The organisations will be tasked with developing four transitions ECHO (Extension of Community Healthcare Outcomes) networks and have a far reaching and transformational impact on transitional care. Project ECHO is a methodology to improve care by gathering a community of practice together for learning, peer mentoring and support. The project aims to bring together regional children’s and adult hospices, local NHS service providers, educators, young people and volunteer groups to build a community based Transition Community of Practice.


We suggest that your Project ECHO Implementation Team comprise of:

  • An ECHO lead/ champion (can be the same individual or separate to the facilitator) to promote and seek engagement in your network.
  • A Facilitator to provide the facilitation of your ECHO network sessions
  • An In-house content expert or specialist to deliver the education in your ECHO network sessions (Optional)
  • An Administrator / Coordinator
  • IT Co-ordinator/Technician
  • (seeking three-four members in total)    
All applicants must be prepared to attend three full days of immersion ECHO Training in London between 3 - 5 December 2019.
